Staff mistakenly believed Pat had a DNR order and ceased resuscitation efforts. It was later discovered they had looked at the wrong patient’s records—a 90-year-old man’s.

The hospital acknowledged the error and made changes to prevent such mistakes in the future. However, the loss devastated Pat’s family, who described the failure of the system she dedicated her life to as catastrophic.

 

Pat’s son, John, expressed disbelief at how she was failed by the very doctors who pledged to do no harm. The coroner’s investigation highlighted several lapses in care, including failing to correctly identify Pat’s records and not following proper medical procedures.

 

Pat’s death serves as a painful reminder of the importance of ensuring patient safety in hospitals. The hospital has since implemented changes to prevent similar incidents and expressed profound regret for the tragedy.
